The brochure/application includes an Enrollment Form with coverage/premium options, payment information and instructions on how to enroll.  The parent/guardian should mail the completed Enrollment Form directly to Sportunderwriters.com Inc, 2047 Saranac Avenue, Suite 201, Lake Placid, NY 12946 along with their check or money order for the appropriate payment made payable to SportInsurance.com, all as stated on the Enrollment Form.  There is also a Medical Claim Form and claim procedures included in the pdf, however as noted above, they also provided the attached separate updated claim form with instructions.
